We proudly offer culturally sensitive homemaking and assisted living care services to Elders / Seniors living independently. We continue to work with local regional agencies to strengthen partnerships so that together we can address the needs of our Indigenous community members.

Barrie & Surrounding Region

Includes Midhurst, Springwater Township & Angus

Indigenous Care Coordinator Services/ Homemaking
For 55 + and adults with age-related health conditions and/or disabilities who self-identify as First Nations, Métis or Inuit.

Services include home-making and assisted living: 

  • Light housekeeping / Homemaking
  • Laundry
  • Meal preparation
  • Social visiting
  • Local trips to shopping centres/banking
  • Telephone reassurance calls

Dementia Supports
Dementia supports include:

  • Education to increase awareness to support earlier identification and reduce stigma of dementia
  • Increase community support services to Indigenous families managing dementia

Long Term Care Liaison Supports
Long term care liaisons services include:

  • Friendly visits: In home visits, assisting with shopping needs and other daily living activities.
  • Security / wellness checks
  • Checking in on the safety and health of isolated /  elderly clients through regular weekly telephone calls and/or home visits.

Additional Liaison Supports: 

  • Advocate on health and related social issues
  • Make referrals to appropriate service providers
  • Identify educational opportunities for clients through workshops, seminars etc. 
  • Assist clients with access to Non-insured Health Benefits, Ontario Disability Support Program and more

Aging@Home Transportation
Helping our Elders Get Places

  • Must be a registered client of the Long Term Care Program.
  • Reliable transportation for elders 55+ and those living with long term illnesses 
  • Assist with attending medical appointments, cultural activities/events, and to see traditional healers
  • Clients are asked to book appointments for transportation in advance
  • We require a minimum 2 week notice for out-of-town trips and request that any in town trips are booked in the same timeframe
  • Scheduled appointments are on a first come, first served basis. We are not able to provide transportation service on an emergency basis. 
  • Please note that we offer standard wheelchair accessible rides, but we are not bariatric wheelchair accessible.

Aging at Home (York Region)
Provides services to those 45+ who identify as First Nation, Metis, Inuit who live in York Region and South Simcoe offering the following services:

  • Home visits/companionship
  • Light housekeeping
  • Transportation (to medical appointments, grocery stores, pharmacy, group programs, etc.)
  • Congregate dining/feasts
  • Culturally based group programs (e.g. Traditional crafts, cooking, drum circles)

We also provide access to the following Indigenous supports:

  • Elders, healers, and knowledge keepers
  • Cultural teachings
  • Traditional ceremonies
  • Regional social gatherings

Senior’s Cultural Coordinator

  • Provide traditional teachings and ceremonies in a safe and acceptable environment to staff, caregivers, community service providers and other community members as needed
  • Organize and facilitate cultural workshops, ceremonies, healing circles and spiritual gatherings for staff, families, caregivers and alternative care providers
  • Coordinate access to Elders and traditional healers to meet the needs of the community
  • Ensure that children and families receive appropriate cultural programming as requested
  • Act as a resource to ensure that cultural knowledge and practices are understood and maintained
  • Foster connections and wellbeing through cultural resources and learning
  • Provide a safe space for wellbeing circles
  • Provide advocacy and referrals to other programs designed to maintain individual personal health
  • Provide traditional meals and maintain Indigenous food sovereignty and its importance in community
  • Manage cross cultural trainings for outside organizations to further their knowledge
  • Provide cultural resources to individuals

Midland & Surrounding Region

Indigenous Independent Living Care / Homemaking
Culturally sensitive care for adults 55+ who are aging at home and who self-identify as First Nations, Métis or Inuit. Homemaking and assisted living services include:
